Description of the project offered:

The overall aim of the project is to develop an outpatient care system for elderly people who suffer from dementia, as well as diabetes.

 Target outcome:

a) Personalised Guidance Services for lifestyle management and disease prevention   or

c) Personalised Services for Independent  Living and Active Ageing

PROJECT DESCRIPTION
Proposal Outline: 

The intended solution

-          consists of a combination of hard- and software

-          is able to monitor the patient status and to give proper information and advices according to the current situation of the patient

-          shall engage patients themselves, support nursing relatives and help attending physicians and nursing staff to keep updated and to interfere whenever necessary

-          The intended solution will be based on sensor technology – a mobile device will constantly record the patients health conditions and behavioral patterns

-          A software-device will be used to analyze this data material in order to create a real time patient’s status report on which health-care advices and recommendations of action will be based

-          The project will emphasize the usability of the solution by focusing on human-computer interaction

-          A very important role within the project will be requirements from the practice of outpatient nursing care. Therefore stakeholders interests and demands will be guiding the project right from the beginning

-          As a by-product of the project a long-term study of the interdependencies between diabetes and dementia will be initiated

-          Interface-development shall take place in order to include data from existing solutions in the field of home-based care
